Appendix A - Screening Requirements Matrix - Overview of screening and training requirements
Appendix B - Volunteer Application Form - On an annual basis, all of Biathlon Saskatchewan’s directors, officers, employees, and committee members will complete a Declaration Form (Appendix B) disclosing any real or perceived conflicts that they might have. Declaration Forms shall be retained by Biathlon Saskatchewan for as long as required.
Appendix C - Screening Disclosure Form - Disclosure of any past convictions, charges, sport disciplinary measures or sanctions. Once every three years
Appendix D - Screening Renewal Form - Acknowledges no changes to VSC, EPIC or SCREENING DISCLOSURE. Must be done annually
